Students will need to complete the following health exams in accordance with Illinois Department of Public Health rules.聽 Have your health care providers fill out the attached forms.聽 They will be due September 30th.

Medical exam form. Required for all 小黄书 entering聽PreK, Kindergarten, 1st grade or 6th grade, as well as for all 小黄书 who are enrolling in school in Illinois (public or private) for the first time.

Dental Examination form. Required for all 小黄书 entering Kindergarten, 2nd grade or 6th grade.

Eye Examination form. Required for all 小黄书 entering Kindergarten.

Even if your child does not need a new examination this year, please bring us copies of the forms from their most recent examinations.
